How To Register As A Patient
People who wish to register as a patient at this surgery must all live within the surgery catchment area, see map on the wall by reception. If you do live in our area please present your NHS Medical Card at reception and the receptionist will complete the necessary registration paperwork for you. If you do not have medical card you will be required to complete a registration form. If you cannot provide a previous address and the name of your previous doctor we will not be able to process your registration. Patients who have recently entered the U.K. must provide the date of entry.
In accordance with national requirements all patients will be required to complete a form detailing their ethnic origins.
If you have a carer, relative or friend that you wish us to deal with us on your behalf please advise the receptionist who will provide a form giving your authorization for them to collect prescriptions etc on your behalf. This information will be entered onto your medical records and will stay there until you cancel this authorization.
We also require current home and mobile telephone contact numbers.
In the event of us declining to accept the registration of any patient that patient will be provided with the reasons for us not accepting their registration in writing eg living out of our area or previously removed from our medical list for any reason.
From time to time the Central Manchester PCT will write to all patients, to check that they are still resident in Manchester. If patients do not respond to these letters they will be considered to have moved out of the area and will be removed from our medical list. It is important that patients keep us advised of address changes otherwise the PCT will write to an old address, will receive no reply and the patient is removed by them from our list of registered patients. In the event of that happening, you may then have to find another GP as we may not be able to re-register you.
